We strive to make your dental experience as easy as possible.

For your convenience, we accept American Express, Discover Card, MasterCard and Visa as well as
ATM cards.

Drs. Mazzetti Financial Policy Regarding Insurance

Drs. Mazzetti dental office is committed to providing the best quality dental care and to make you visit with us go as easily as possible. To do this, we will as a courtesy, submit your insurance for you providing you have given us all the necessary information we need. Please be aware however, that this does not dismiss you from your financial responsibility. At the time of your appointment we will estimate your portion for your insurance co pay. All estimated co pays are due at the appointment. We will do our best to give you an estimate of what your insurance will pay but if for any reason your insurance company does not pay  or does not pay in a timely manner, payment in full is due from you. We will allow 45 days for your insurance company to pay on a claim. We therefore recommend that you follow up with your insurance.

Please remember that your policy is a contract between you and your insurance company. We are not a party to that contract and will therefore not become involved in disputes between you and your insurance company regarding deductibles, covered charges and usual and customary fees, other than to provide factual information as necessary. We also are not responsible for knowing all of your plan policies, provisions and limitation so it is in your best interest to know what your insurance covers.

Your acceptance of this policy also authorizes Drs. Mazzetti to release information relating to any dental claims to your insurance carrier and to have dental benefits paid directly to our office.

Payment in full is required at time of service if you do not have dental insurance. If you have dental insurance please read the information below regarding filing of insurance claims and payments.

There will be a $50.00 charge for failed appointments or appointments canceled or rescheduled without 24 hours advance notice.

More than 2 consecutive failed appointments or habitually canceled appointments could result in your dismissal from the practice.

Past due accounts may be subject to a 38% collection fee if referred to a collection agency.
